Youth Sports Camp Waiver and Release of Liability

I, the undersigned parent or legal guardian of the participant named above, understand and acknowledge that

participation in any sports or athletic program involves inherent risks of injury, illness, or other harm. By

signing below, I agree to the following on behalf of myself and my child:

1. Assumption of Risk

I voluntarily assume all risks associated with my child's participation in the 4 Hills Athletic Performance youth

sports camp, including but not limited to falls, contact with other participants, the effects of weather, and

facility conditions.

2. Release of Liability

I release and hold harmless 4 Hills Athletic Performance, its officers, staff, coaches, volunteers, and partners

from any and all liability, claims, or demands for injuries or damages arising from my child's participation,

whether caused by negligence or otherwise, to the fullest extent permitted by law.

3. Medical Authorization

In the event of an injury or medical emergency, I authorize 4 Hills Athletic Performance staff to secure

emergency medical care for my child. I understand that I am financially responsible for any medical treatment

provided.

4. Behavioral Expectations and Dismissal

I understand that participants are expected to behave respectfully and follow all camp rules. 4 Hills Athletic

Performance reserves the right to dismiss any participant whose behavior compromises the safety or

experience of others.

5. Media Release

I grant permission for my child to be photographed or recorded during camp activities and for those images to

be used in promotional materials, social media, and website content for 4 Hills Athletic Performance.

6. COVID-19 & Communicable Diseases

I acknowledge the ongoing risks associated with communicable diseases, including COVID-19, and agree that 4 Hills Athletic Performance cannot guarantee an environment free of exposure.
