ALL IN TRAINING WRESTLING CAMPPARTICIPANT WAIVER, RELEASE OF LIABILITY & MEDICAL AUTHORIZATION

Camp Dates: August 21–23

IMPORTANT NOTICE

Participation in wrestling activities involves inherent risks, including but not limited to physical injury, illness, skin infections, permanent disability, paralysis, and death. By signing this waiver, the parent/legal guardian acknowledges and accepts these risks.

SKIN CHECK POLICY

All athletes may be subject to a skin check prior to participation.

Any athlete displaying signs of a contagious skin condition, infection, lesion, ringworm, impetigo, staph infection, or any other condition deemed unsafe by camp staff may be removed from participation without refund.

MEDICAL AUTHORIZATION

In the event of injury, illness, or emergency, I authorize ALL IN Wrestling Camp staff, athletic trainers, emergency medical personnel, and licensed healthcare providers to obtain medical treatment for my child as deemed necessary.

I understand that I am solely responsible for all medical expenses incurred as a result of treatment.

ASSUMPTION OF RISK

I understand that wrestling is a contact sport involving physical exertion and inherent risks. I voluntarily assume all risks associated with participation in camp activities, including but not limited to:

• Wrestling instruction
• Drilling
• Live wrestling
• Conditioning activities
• Recreational activities
• Travel to and from camp activities

I acknowledge that injuries may result from my child's actions, the actions of others, equipment, facilities, or unforeseen circumstances.

RELEASE OF LIABILITY

On behalf of myself, my child, my heirs, representatives, and assigns, I release and forever discharge:

• Levi Jones
• ALL IN Wrestling
• ALL IN Training Wrestling Camp
• Kittitas High School
• Kittitas School District
• Camp staff
• Coaches
• Volunteers
• Clinicians
• Employees
• Facility owners and operators

from any and all claims, demands, damages, causes of action, liabilities, costs, or expenses arising from participation in camp activities, except where prohibited by law.

This release includes claims arising from personal injury, illness, property damage, disability, or death.

CAMP POLICIES

I understand and agree that:

• Camp fees are non-refundable unless otherwise approved by camp management.
• Camp staff reserve the right to remove any participant for unsafe behavior, misconduct, bullying, harassment, or violation of camp rules.
• No refunds will be issued for dismissal due to misconduct.
• Camp participants must follow all safety instructions provided by camp staff.

PHOTO & VIDEO RELEASE

I grant ALL IN Wrestling permission to photograph, video record, and use images or recordings of my child for promotional, educational, marketing, and social media purposes without compensation.

ACKNOWLEDGMENT

I have carefully read this Waiver, Release of Liability, Assumption of Risk, and Medical Authorization Agreement.

I understand its contents and understand that I am giving up certain legal rights by signing it.

I voluntarily agree to all terms and conditions contained herein.
