Faizon Brandon Football Camp Waiver

I hereby consent to my child’s participation in the Faizon Brandon Youth Football Camp (“FBYFC”) to be held at Anderson County High School on July 25, 2026. I have no knowledge of any physical impairment that would be affected by my child's participation in the FBYFC. I understand that by the very nature of athletics, football and related activities cause a risk of potential injury. I further understand and agree that regardless of the caution exercised by a player, coach, or anyone involved in said activity, there is a risk of injury. I represent and warrant that my child is physically and mentally able to participate in the FBYFC. I hereby assume all risks associated with participation in the FBYFC and agree to hold harmless, Faizon Brandon, the FBYFC, Jus1Fa1zon, LLC its directors, officers, employees, agents, representatives, coaches, volunteers, school officials, and athletic trainers (“Camp Officials”) from and against any and all claims, demands, losses or liability of any kind or nature which are in connection with injuries suffered to my child while participating in the FBYFC. I also hereby authorize the staff of the FBYFC to act according to their best judgment in any emergency requiring medical attention and I hereby waive and release the Camp Officials from any and/or all liability for any injuries or illnesses incurred while at camp or in transportation to a medical facility.
