Basketball Training Waiver & Release of Liability

I, the undersigned, understand that participation in basketball training involves physical activity and carries a risk of injury. I voluntarily assume all risks associated with participation.

I hereby release and hold harmless the coach, trainers, facility, and any affiliated parties from any and all liability, claims, or demands arising from injuries, losses, or damages that may occur during training sessions.

I certify that I am physically fit to participate and have no medical condition that would prevent safe involvement.

Lastly, I give Coach Rod permission to use video and images from the clinic to advertise to potential, future participants.
