Assumption of Risk

I understand that participation in athletic training, practices, competitions, travel, strength and conditioning activities, and related clinic activities involves inherent risks, including but not limited to physical injury, illness, permanent disability, paralysis, or death. I voluntarily choose to allow my child/participant to participate in all camp activities, fully understanding these risks.

In consideration of being allowed to participate in the TIGA After School Tennis Enrichment Program , I, on behalf of myself, my child, and our heirs and assigns, hereby release, waive, discharge, and covenant not to sue TIGA Inc., its directors, officers, employees, coaches, contractors, volunteers, affiliates, sponsors, host facilities, and representatives from any and all claims, demands, causes of action, damages, or liabilities arising out of or related to participation in the camp, including but not limited to those caused by negligence, to the fullest extent permitted by law.

I authorize TIGA Inc. staff and designated medical personnel to obtain and provide emergency medical treatment for my child/participant if I cannot be reached. I understand that I am financially responsible for any medical treatment provided.

I certify that my child/participant is covered by valid health insurance. I understand that TIGA Inc. does not provide medical insurance coverage for participants and that all medical expenses are my responsibility.

I grant permission for photographs and/or video of my child/participant to be used for promotional, educational, or marketing purposes by TIGA Inc.

This agreement shall be governed by and interpreted in accordance with the laws of the jurisdiction in which the program is held.

I have read and fully understand this Program Waiver & Release of Liability. I voluntarily sign this agreement, acknowledging that it is legally binding.

This waiver must be completed and signed before participation in any TIGA Inc. program activities.

Student Code of Conduct

Event Name: TIGA After School Tennis Enrichment Program

Event Type: Clinic

Clinic Dates: September 28 / October 5 and 26 / November 2, 9 and 16

Location: King School, 1450 Newfield Ave, Stamford, CT 06905 

Purpose

The purpose of this Code of Conduct is to ensure a safe, respectful, and high-performance environment for all tennis student-athletes, coaches, staff, and guests. Participation in this clinic is a privilege, and all students are expected to represent the clinic with integrity, discipline, and respect at all times.

General Student Expectations

All kids are expected to:

1. Show Respect 

2. Treat fellow teammates, coaches, staff, and facility staff with

respect at all times.

3. Harassment, bullying, discrimination, inappropriate language, or

disrespectful behavior will not be tolerated.

Disciplinary Actions

Failure to follow this Code of Conduct may result in:

1.Immediate dismissal from the program without refund.

Acknowledgment & Agreement

I understand that participation in this TIGA After School Tennis Enrichment Program requires adherence to this Student Code of Conduct. I agree to follow all rules, expectations, and instructions.

I have reviewed this Code of Conduct with my child and understand the expectations and consequences.

This Code of Conduct is subject to updates at the discretion of Tiga Inc. administration.
