Waiver and ReleaseBy signing below, I acknowledge and agree to the following:

Assumption of Risk: I understand that participating in the basketball shooting clinic with Coach Rod Mosby may involve physical activity and carries certain inherent risks, including the risk of injury. I assume full responsibility for any personal injury, illness, or property damage that may occur.

Release of Liability: I hereby release and hold harmless Coach Rod Mosby, the Beebe Athletic Club, and all employees, agents, or representatives associated with the clinic from any and all liability, claims, or demands arising from my participation in the clinic, whether caused by negligence or otherwise.

Code of Conduct: I agree to respect all participants, staff, and property at the clinic. I understand that any disruptive behavior or property damage caused by me will result in my immediate removal from the clinic without refund.
